The University of St. Thomas Athletic Sports Medicine Department uses Athletic Trainers Systems (ATS®) to manage each student athlete’s pre-participation forms and medical records.  All individuals that will be participating in or trying out for a sport will be required to have their forms completed before they are allowed to tryout or practice/participate.

Below are two instruction documents, one for Freshman/Transfer Athletes and one for Returning Athletes.  Please choose the document that applies to you.  The Freshman/Transfer Athletes instructions will talk you through the steps of creating an account and completing your forms.

All freshman/first year athletes/transfer athletes will be required to have a physical.  The physical most be completed within 6 month of submitting pre-participation paperwork to the University of St. Thomas.   This includes all athletes that are trying out for a team for the first time.  Physicals from the following providers meet institutional requirements.  Physicians (MD or DO), Physician’s assistant (PA-C) or Nurse Practitioner.

Sickle Cell Trait Information

On the Sickle Cell Verification/Waiver form you will be asked to either provide information on your sickle cell status or you will be given the chance to sign a waiver.  Directions on how to proceed are provided on the Freshman/Transfer Instruction sheet as well as on the Sickle Cell Verification/Waiver  form on line.
